CLEARANCE PROCEDURE TO PARTICIPATE IN SPORTS AT DANVERS HIGH SCHOOL & HOLTEN-RICHMOND MIDDLE SCHOOL

Each student-athlete is required to have a physical examination annually (13 months) to be able to participate in sports. This is the policy of the Danvers Public School Committee and the Massachusetts Interscholastic Athletic Association (MIAA).  The physical form required is the MIAA Sports Candidate Medical Questionnaire, but proof on proper physician letterhead is acceptable. Students become ineligible when the physical runs out.  Please schedule physicals accordingly and always remember to bring updated physicals to the Athletic Office. Other forms required are the State Law Concussion Form, the Pre-Participation Head Injury/Concussion Reporting Form, and the parent consent/permission form.  All these forms are included as part of the seasonal student-athlete online registration.  Registration payment must also be submitted online at the time of registration. (Please note that in addition to the student-athlete’s online registration, a HARD COPY of the student-athlete’s physical MUST still be submitted to the Athletic Department).

The student’s updated physical, online registration, and payment must be received by the Athletic Department, two weeks prior to the first day of practice/tryouts.  Please check the website, and the local newspaper for specific dates and times for sign-ups and try-outs for your sport.

No student-athlete will be allowed to participate in any school sport (tryouts/practice or game), receive equipment/uniform until cleared from the Athletic Office.  Coaches can not collect any forms or fees and clear student-athletes to participate.

 

Remember…  No state law concussion form = NO CLEARANCE

No pre-participation head injury/concussion reporting form = NO CLEARANCE

No proof of physical  =  NO CLEARANCE

No permission form  =  NO CLEARANCE

No activity fee  =  NO  CLEARANCE

NO CLEARANCE  =  NO PARTICIPATION

  • Important Medical Notation:  If your child requires the use of an Epi-Pen or inhaler the parent should speak to the coach before the first practice.
